Below are the documents that must be submitted:

FOR NEW APPLICANTS:

0 Duly accomplished license application form with corresponding license fee
0 Contract of employment of radiologist and radiologic/x-ray technologist
0 Certificate of training of doctor if he/she is not a fellow of Philippine College of Radiology
0 Photocopy of PCR certificate and PRC license of radiologist (if diplomate or fellow)
0 Photocopy of PRC license of radiologic/x-ray technologist
0 Certificate of training in radiation protection of radiologic/x-ray technologist if he/she acts as Radiation Safety Officer
0 Photocopy of business/mayor’s permit and SEC registration certificate

FOR RENEWAL:

If compliance is required:

0 Duly accomplished license application form with corresponding license fee
0 Photocopy of latest license certificate
0 Contract of employment of radiologist or radiologic/x-ray technologist if new one is hired together with photocopy of PCR certificate of the radiologist or PRC license of radiologic/x-ray technologist
0 Copy of latest radiation protection survey and evaluation report
0 Photocopy of business/mayor’s permit

If recommended for renewal:

0 Duly accomplished license application form with corresponding license fee
0 Photocopy of latest license certificate and radiation protection survey and evaluation report
0 Photocopy of business/mayor’s permit

Any of the following whenever applicable:

0 Notarized certificate of compliance
0 Proof of compliance (pictures, receipts, charts, logbooks, etc.)
0 Contract of employment of radiologist or radiologic/x-ray technologist
0 Documents required by the BHDT health physics team
